Key Responsibilities\n

• Provide direct clinical services to clients diagnosed with major mental illnesses or severe emotional disorders, conducting assessments and facilitating individual, group, and family therapy sessions.

• Collaborate effectively with a multidisciplinary team to establish common goals and enhance client care coordination, ensuring effective communication across all involved parties.

• Conduct brief, evidence-based screenings and determine the need for further comprehensive assessments when necessary.

• Develop and implement integrated care plans that address clients' presenting problems, ensuring access to necessary linked services.

• Monitor treatment plans, documenting the progress of clients and making adjustments as required in response to their needs.

• Facilitate crisis interventions, providing immediate support and guidance during critical situations.

• Participate in agency committees focused on quality improvement initiatives and contribute to enhancing service delivery standards.

• Provide on-call coverage during and after program hours as needed to maintain continuity of care.

• Perform administrative duties including timely completion of clinical documentation and data submissions to comply with organizational standards.
