Key Responsibilities\n

• Provide counseling and emotional support to seniors and their families, assisting them through transitions, such as health challenges or loss.

• Work in close conjunction with an interdisciplinary team of nurses, caregivers, and healthcare professionals to enhance overall patient care and outcomes.

• Regularly monitor clients’ well-being through home visits, assessing living conditions and addressing any safety concerns that may arise.

• Foster effective communication among family members involved in the care process, mediating conflicts when necessary.

• Educate clients and their families about community resources and support systems that can provide additional help and encourage social engagement.

• Keep detailed and accurate documentation of client assessments, care plans, and progress notes to comply with organizational standards.

• Stay abreast of current social work practices, policies, and legal regulations to uphold the highest ethical standards in all interactions.

In this role, you will primarily serve a senior population, many of whom are experiencing complex health issues and require both emotional and practical support. Qualifications\n

• A Master’s degree in Social Work from an accredited institution is required.

• Current and valid state licensure as a Licensed Master Social Worker (LMSW) is mandatory.

Compensation and Benefits\n
