Work Environment

This position is primarily based onsite, as our therapists travel to patients' residences to deliver tailored care. Key Responsibilities

As a Home Care Physical Therapist, you will:

• Assess, plan, and implement effective treatment programs tailored to the specific needs of your patients as referred by their physicians.
• Deliver physical therapy services in the patients' homes, helping them to regain movement and strength following various health challenges, including surgeries, injuries, and chronic conditions.
• Utilize a variety of therapeutic techniques and modalities, ensuring each therapy session adheres to the physician's plan of care.
• Educate patients and their caregivers on exercises, proper body mechanics, and other strategies for improving mobility and function.
• Monitor progress and modify treatment plans as necessary to ensure optimal outcomes, incorporating evidence-based practices throughout.
• Demonstrate exceptional customer service, fostering positive relationships that encourage patient engagement and adherence to treatment plans.
• Collaborate with a multidisciplinary team to maximize patient outcomes and participate in continuous learning opportunities.
• Meet minimum utilization goals and contribute to the business objectives of the practice.

Patient Population / Client Focus

You will cater to diverse patient populations, treating orthopedic, pediatric, neurologic, and sports medicine cases. Your expertise will allow you to address a spectrum of conditions, from post-operative recovery to chronic pain management, tailoring your approach to each patient's unique circumstances. Building strong, trusting relationships with your patients is crucial, as it empowers them to participate actively in their recovery process.

Qualifications

• A Doctorate degree in Physical Therapy (DPT) from an accredited institution.
• A valid State License in Physical Therapy, ensuring compliance with local regulations.
• A minimum of two years of clinical experience in a relevant healthcare setting.
• Basic Life Support Certification from the American Heart Association.
• Strong written and verbal communication skills, essential for educating patients and collaborating with the care team.
• Proficiency in word processing and computer skills to maintain accurate patient records and documentation.
• Ability to establish and maintain positive relationships with patients, contributing to their emotional and physical wellbeing.

Preferred Experience

• Experience in home healthcare settings or similar environments is highly desirable.
• Familiarity with evidence-based therapeutic practices and techniques across various specialties.

Compensation and Benefits

This full-time position offers a competitive salary range between USD 100,000 and 120,000 annually, reflecting your skills and experience. In addition to a comprehensive salary, we provide:
