Key Responsibilities\n

• Conduct detailed psychosocial evaluations and assessments to determine the needs of seniors and their families.

• Participate in the development and implementation of Individualized Family Service Plans (IFSP) ensuring that care aligns with identified needs.

• Collaborate with interdisciplinary team members to monitor client outcomes and adjust care plans as necessary.

• Provide consistent support and counseling to family members, addressing their emotional and practical needs.
\n
• Administer crisis interventions when required and ensure adherence to all applicable Federal and State regulations.
\n
• Establish rapport with clients, creating a safe and trusting environment conducive to open communication.

• Document case findings and progress notes in accordance with organizational standards and regulatory requirements.

Qualifications\n

• Master's degree in Social Work (MSW) or a related field is required.

• A valid Licensed Clinical Social Worker (LCSW) certification in the state of Georgia is essential.

• Superior verbal and written communication skills with the ability to build rapport effectively.

Preferred Experience\n

Compensation and Benefits\n

This full-time position offers a competitive salary ranging from USD 80,000 to 100,000 annually.
