Key Responsibilities\n

• Perform bio-psychosocial evaluations to identify patients' needs, barriers to treatment, and opportunities for enhanced health outcomes.

• Develop and implement individualized treatment plans utilizing various evidence-based modalities in both individual and group therapy settings.

• Collaborate with medical and support staff to create a cohesive care program that fosters patient engagement and improves adherence to treatment protocols.

• Document social work activities comprehensively in patients' medical records, ensuring compliance with department standards and legal regulations.

• Provide advocacy support by connecting patients and families with community resources, enhancing their support systems and overall well-being.

• Facilitate educational sessions for patients and their families to help them understand their social, emotional, and psychological needs, fostering a sense of empowerment and self-determination.

• Maintain current knowledge of community resources and innovative practices in social work to enhance service delivery.

Patient Population / Client Focus\n

Your work will primarily involve supporting individuals in detox and recovery from substance use disorders. Qualifications\n

• A Master’s Degree in Social Work from an accredited university.

• Current licensure as a Licensed Clinical Social Worker (LCSW) in the state of Arizona.

Compensation and Benefits\n
