Key Responsibilities\n

• Coordinate with local community organizations to ensure that students have access to ongoing support, resources, and services post-discharge.

• Provide crisis intervention and emotional support during critical moments, acting as a vital resource for both students and families.

• Create and implement effective discharge plans that connect students with appropriate social services and community resources.

• Participate in interdisciplinary team meetings to enhance holistic care approaches and improve overall student outcomes.

Patient Population / Client Focus\n

In this role, you will primarily work with students from diverse backgrounds, encompassing a variety of emotional, social, and developmental challenges. You will facilitate services that address issues such as anxiety, depression, trauma, and family dynamics, creating a safe environment for students to express their feelings and experiences.

Qualifications\n

• A Master’s degree in Social Work from an accredited program.

• Active and valid state licensure as a Licensed Clinical Social Worker (LCSW).
