Key Responsibilities\n

As a Licensed Master Social Worker at Optimum Care Counseling, you will:

• Maintain accurate and thorough documentation of assessments, care plans, and client interactions to ensure compliance with ethical and regulatory standards.

Patient Population / Client Focus\n

In this role, you will primarily serve the senior population facing life-limiting illnesses and complex care needs, often in a hospice setting. You will work closely with families to help them understand the resources available to them and promote the best possible outcomes for their loved ones.

Qualifications\n

• Master's degree in Social Work from an accredited institution.

• Current state licensure as a Licensed Master Social Worker (LMSW).

• A minimum of two years of relevant experience in social work, particularly within a hospice, geriatric, or community center setting.

Preferred Experience\n

We value candidates with experience in hospice care, geriatric social work, or familiarity with available community resources in Philadelphia. Experience in conflict resolution and family counseling is also advantageous.

Compensation and Benefits\n

We offer competitive annual salaries ranging from $60,000 to $100,000, commensurate with experience and qualifications. Benefits include:
