Key Responsibilities\n

• Conduct thorough psychosocial assessments to evaluate the emotional, social, and financial needs of patients.

• Collaborate with medical teams to tailor care plans that address the unique psychosocial aspects of each patient’s health situation.

• Provide individual and group counseling services, addressing issues such as adjustment disorders, loss and grief, anxiety, and coping strategies.

• Facilitate crisis intervention to assist patients and families during critical moments, ensuring they receive timely support and resources.

• Develop comprehensive discharge plans and coordinate with community organizations to secure post-discharge services that promote continuity of care.

• Educate patients and their families about treatment options, available resources, and strategies for effective coping and adjustment.
\n
• Maintain accurate and timely documentation in alignment with organizational policies, ensuring confidentiality and compliance with legal standards.

• Participate in community outreach initiatives to enhance awareness of mental health resources and services available through Mountain Springs Wellness Center.

Qualifications\n

• Master's degree in Social Work from an accredited program.

• Current and valid state licensure as a Licensed Clinical Social Worker (LCSW).

Compensation and Benefits\n
