FlexStaff Medical Center is seeking a dedicated and skilled Family Nurse Practitioner to join our dynamic team. Work Environment

Key Responsibilities

• Conduct thorough health assessments and physical examinations of patients, ensuring accurate evaluations.
• Order, perform, and interpret diagnostic tests (such as blood work and imaging) to substantiate diagnoses and shape treatment plans.
• Collaborate closely with a multidisciplinary care team, including physicians and mental health professionals, to design and implement individualized patient care strategies.
• Prescribe appropriate medications and create tailored treatment regimens based on patient assessments.
• Deliver health education and counseling to patients and their families, focusing on prevention, management of chronic conditions, and overall wellness.
• Engage in case conferences and team meetings to discuss patient progress, treatment modifications, and care coordination.
• Document all patient encounters in electronic health records (EHR) accurately and promptly, ensuring up-to-date information is available for all providers involved in patient care.
• Participate in quality improvement initiatives designed to enhance patient outcomes and streamline care processes.

Patient Population / Client Focus

Your expertise will primarily benefit children and adolescents within the school district, alongside their families. You will cater to a wide spectrum of health issues ranging from acute illnesses to chronic disease management, always with an emphasis on health promotion and preventive care. This position allows for impactful relationships with patients, empowering them through education and support.

Qualifications

• A valid, unrestricted Nurse Practitioner license in New York State.
• A minimum of 2 years of experience as a Family Nurse Practitioner.
• Comprehensive knowledge of family medicine practices, healthcare protocols, and current clinical guidelines.
• Exceptional clinical assessment and diagnostic skills, with the ability to think critically and make sound decisions based on clinical findings.
• Adept in the use of electronic health records (EHR) systems to manage patient data and documentation.

Preferred Experience

• Prior work experience in a school-based health setting or community health environment is highly desirable.
