Work Environment

Your workplace will be a vibrant outpatient office, where you will engage directly with patients in a friendly and welcoming atmosphere. The typical schedule is Monday to Friday from 9:00 a.m. to 6:00 p.m., with additional evening clinics as necessary. Key Responsibilities

As a Family Nurse Practitioner at Indigo Total Care, you will:

• Conduct thorough health assessments for patients of all ages, identifying acute and chronic health issues.
• Diagnose medical conditions and develop tailored treatment plans, ensuring effective preventive care and health promotion.
• Provide patient-centered education, addressing lifestyle modifications and existing health concerns.
• Collaborate with community organizations to design and implement health education programs aimed at improving community awareness and engagement.
• Advocate for patients, ensuring their rights are upheld, and facilitate access to necessary healthcare services.
• Coordinate referrals to specialists and follow-up care, ensuring continuity of care for patients.
• Maintain comprehensive medical records in compliance with healthcare regulations and standards.
• Participate in community outreach programs, providing health screenings and education to promote health awareness and access.
• Stay informed on community health trends and evidence-based practices to enhance individual and community care.

Patient Population / Client Focus

You will be working with a diverse patient population, including individuals and families from various backgrounds and socioeconomic statuses. Qualifications

To be successful in this role, you will need:

• A current certification as a Nurse Practitioner.
• An active state nursing license to practice in New York.
• A Master's degree in nursing from an accredited institution.

Compensation and Benefits
