Key Responsibilities

• Conduct thorough assessments of patients’ medical histories, performing physical examinations and diagnostic tests, including laboratory and imaging studies, to form a differential diagnosis.
• Order and interpret diagnostic studies, developing comprehensive care plans tailored to individual patient needs.
• Facilitate hospital admissions, transfers, and discharges, ensuring that patients receive the appropriate inpatient and outpatient accommodations, including home care when necessary.
• Provide education and counseling to patients and their caregivers on disease processes, treatment options, and coping strategies to support effective self-management.
• Perform phone triage for acute clinical issues, conducting assessments and prescribing treatments as per protocol.
• Engage in discharge planning and follow-up care coordination to optimize patient outcomes and continuity of care.
• Participate actively in multidisciplinary team meetings to discuss patient progress, treatment updates, and refine care strategies.
• Maintain accurate and timely patient records in compliance with regulatory standards and organization policies.

Qualifications

• Master of Science in Nursing (MSN), Doctor of Nursing Practice (DNP), or a graduate of an accredited Nurse Practitioner program.

• Active and unrestricted license to practice as a Nurse Practitioner in the state of New York.
• Eligibility to work in the United States.

Preferred Experience
