Full-Time Nurse Practitioner for Home Care Services

Work Environment

This position is based onsite in the homes of patients in the Utica area, allowing you to provide personalized care tailored to individual needs. You will collaborate closely with an interdisciplinary care team, including primary care providers and specialists, to ensure comprehensive patient management. The NP will be expected to maintain a presence in assigned facilities to ensure an adequate level of face-to-face patient care.

Key Responsibilities

• Develop and implement individualized treatment plans, effectively communicating these plans to patients, caregivers, and other healthcare providers.
• Order, interpret, and act upon laboratory and diagnostic tests to monitor and optimize treatment based on patient responses.
• Maintain accurate and compliant documentation in our electronic medical record (EMR) system to ensure seamless patient data management.
• Collaborate with local and regional clinical leadership to contribute to the organization’s operational goals and the advancement of quality care initiatives.
• Participate in initiatives that emphasize value-based care, improving both health outcomes and patient satisfaction.
• Provide health education to patients and families to promote healthy lifestyle choices and chronic disease management.
• Assess, diagnose, and manage both chronic and acute medical conditions, ensuring timely intervention and follow-up.
• Actively engage in family conferences to assist in advanced care planning and to address specific patient needs and preferences.
• Coordinate care with various specialists and incorporate telemedicine options when necessary to enhance patient access to comprehensive care.
• Respond to recommendations from pharmacists regarding medication management and collaborate with pharmacy services for optimal patient outcomes.
• Complete all required regulatory visits timely and comprehensively, including monthly, quarterly, and annual evaluations.

Patient Population / Client Focus

This position primarily focuses on providing care for senior patients, many of whom experience complex health conditions and a high need for individualized attention. You will work closely with low-income populations, ensuring they receive equitable access to quality healthcare services, while addressing their particular concerns and health challenges.

Qualifications

• Master of Science in Nursing (MSN), Doctor of Nursing Practice (DNP), or completion of an accredited Nurse Practitioner Program.
• Current national certification from the American Association of Nurse Practitioners (AANP) or the American Nurses Credentialing Center (ANCC).
• An active, unrestricted nurse practitioner license in New York State.
• An active DEA license for prescribing medications.
• Authorization to work in the United States.
• Demonstrated ability to work flexibly within varying schedules and environments.

Preferred Experience

• Experience in home care, skilled nursing facilities, or geriatric care settings is highly desirable.
• A strong passion for serving senior and low-income populations and a commitment to compassionate, patient-centered care.

Compensation and Benefits
