Key Responsibilities

• Conduct detailed health assessments and physical examinations, developing individualized treatment plans that meet the unique needs of patients.
• Order, analyze, and interpret diagnostic tests such as blood work and imaging studies to inform clinical decision-making.
• Prescribe appropriate medications and implement therapeutic interventions, monitoring patient responses for ongoing effectiveness.
• Educate patients and their families on health promotion techniques, disease prevention strategies, and the management of chronic illnesses like diabetes and hypertension.
• Maintain thorough and accurate medical records in compliance with regulatory requirements, utilizing electronic health record (EHR) systems to facilitate documentation.
• Collaborate effectively with interdisciplinary team members, including physicians, social workers, and dietitians, to ensure seamless patient care and facilitate timely referrals.

Patient Population / Client Focus

In this role, you will serve a diverse patient population, including children, adults, and the elderly, addressing a wide array of health concerns from acute illnesses to chronic disease management. You will take a lead role in supporting underrepresented communities, ensuring equitable access to healthcare resources and education while working towards improving health disparities in the area.

Qualifications

• A Master’s or Doctoral degree in Nursing from an accredited institution.
• Current licensure as a Nurse Practitioner in the state of New York.
• Certification in a recognized Nurse Practitioner specialty area is required.
• Strong clinical assessment skills and the ability to diagnose complex medical conditions effectively.
• Exceptional communication skills for effective patient and team interactions.

Preferred Experience
