Key Responsibilities

• Conduct comprehensive health assessments, including detailed histories and thorough physical examinations for adult patients.
• Diagnose and manage both acute and chronic medical conditions autonomously, adhering to New York state regulations for nurse practitioners.
• Order, interpret, and follow up on laboratory and radiology tests, as well as other appropriate diagnostic studies to inform treatment decisions.
• Develop and implement individualized treatment plans that include pharmacologic and non-pharmacologic interventions.
• Monitor patient progress, establish clear health outcomes and goals, modify treatment plans as necessary, and communicate effectively with patients about their health status.
• Provide preventative care, wellness education, and lifestyle guidance to promote health among patients and their families.
• Collaborate with other healthcare professionals to optimize treatment and interventions and ensure coordinated care.

Patient Population / Client Focus

This role focuses primarily on adult patients residing in an assisted living facility, many of whom may have chronic health conditions requiring ongoing management. Your expertise will aid in enhancing their quality of life and managing health intricacies, ensuring they receive both medical and emotional support in a familiar setting.

Qualifications

• A Master's degree in Nursing and current licensure as a Registered Professional Nurse and Nurse Practitioner in New York State is required.
• Must possess Nurse Practitioner Board Certification and current Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) certifications.
• Exceptional clinical skills and the ability to effectively communicate both verbally and in writing are essential for this role.

Preferred Experience

• We prefer candidates with a minimum of three years of advanced clinical experience in an internal medicine setting, particularly those familiar with the nuances of caring for adult patients.
