Key Responsibilities

• Collaborate with physicians and other healthcare professionals to develop and implement individualized care plans for patients across various health conditions.
• Conduct thorough patient assessments, including obtaining medical, physical, and psycho-social histories, and perform comprehensive health evaluations tailored to age and medical history.
• Carry out preventive screening procedures that are relevant to the patients’ demographics, ensuring early detection and intervention of health issues.
• Utilize patient history, physical examination findings, and clinical assessments to create accurate differential diagnoses.
• Design and deliver effective educational resources and plans that empower patients regarding their health status and management.
• Prescribe medications, apply therapeutic methods, and communicate non-pharmacologic care strategies that adhere to established clinical guidelines.
• Participate in regular team meetings to review patient progress and discuss care strategies, ensuring ongoing enhancement of service delivery.

Patient Population / Client Focus

The Nurse Practitioner will serve a diverse patient population with varying medical needs, including chronic disease management, acute health concerns, and preventive care. Qualifications

• A Master’s Degree in Nursing with a concentration as a Nurse Practitioner is essential.
• A current, active Registered Nurse (RN) license in the state of New York.

• Proven proficiency with electronic health record (EHR) systems and medical technology for effective patient management.

Preferred Experience

Compensation and Benefits

The position offers a competitive salary ranging from USD 150,000 to 160,000 annually, commensurate with experience and qualifications.
