Work Environment

The position is based onsite, where you will work closely with a multidisciplinary team in our Home Care setting. Key Responsibilities

• Develop and maintain precise patient medical records, ensuring compliance with all regulatory standards and facilitating seamless communication with our healthcare team.
• Collaborate with diverse healthcare professionals to create individualized care plans for patients, ensuring a holistic approach to treatment that incorporates physical, emotional, and social health considerations.
• Engage in regular patient follow-ups, conducting routine check-ups and actively monitoring treatment progress for patients with chronic cardiovascular conditions.
• Prescribe appropriate medications and therapies following established clinical protocols, assuring adherence to best practices in treatment to enhance patient safety and effectiveness.
• Conduct thorough assessments and manage chronic cardiovascular conditions such as hypertension, heart failure, and arrhythmias, utilizing evidence-based treatment protocols to optimize patient outcomes.

Patient Population / Client Focus

In this role, you will serve a diverse population of patients, focusing on individuals with various cardiovascular conditions. This includes patients experiencing acute cardiac episodes, those living with chronic conditions, and individuals requiring long-term management strategies. Your expertise will play a vital role in improving their health outcomes and overall quality of life.

Qualifications

• A Master’s Degree in Nursing from an accredited institution.
• A valid Nurse Practitioner license in the state of New York.
• A minimum of two years of direct experience in cardiology or a related healthcare field, demonstrating a strong understanding of cardiovascular diseases.
• Exceptional ability to work both independently and collaboratively within a multidisciplinary healthcare team.

Preferred Experience

Compensation and Benefits
