Key Responsibilities

• Conducting thorough assessments to manage both physical and psychological health issues of patients, providing holistic care.
• Identifying normal and abnormal clinical findings through careful observation and physical examination.
• Developing individualized treatment plans based on assessments, patient history, and current clinical guidelines; regularly reassessing and modifying plans as required.
• Evaluating patients’ responses to medications and treatments, documenting outcomes, and making necessary adjustments.
• Prescribing immunizations according to established health guidelines.
• Engaging in effective communication with physicians and other healthcare team members to ensure clarity and continuity in patient care.
• Maintaining an accurate Problem/Medication List for each patient at every encounter.
• Ensuring the completeness, accuracy, and timeliness of documentation for all patient interactions, in compliance with regulatory standards.

Qualifications

• A Master's degree from an accredited Nurse Practitioner program.
• Current Registered Nurse (RN) License in New York State.
