Work Environment

The practice is located in Utica, New York, in a modern outpatient office setting where you will work onsite alongside a dedicated and collaborative healthcare team. The team includes physicians, fellow nurse practitioners, physician assistants, medical assistants, registered nurses, care managers, and a dedicated medical scribe. Our state-of-the-art facility is designed to promote seamless communication and collaboration among healthcare providers, ensuring that each patient receives comprehensive care tailored to their needs.

Key Responsibilities

• Deliver exceptional primary care services, including conducting thorough office visits for routine and acute health issues.
• Administer annual wellness visits and perform health risk assessments that consider a holistic view of health, ensuring detailed documentation for effective patient care.
• Coordinate care with specialists, testing facilities, and agencies to provide integrated health services.
• Lead initiatives in population health to ensure preventive measures, such as scheduling evidence-based breast cancer screenings for female patients.
• Assist the care team with phone triage and outreach efforts to maintain strong patient relationships.
• Educate patients about their conditions, care plans, and treatment options to empower them in their health journey.
• Occasionally conduct home visits to reach patients who may require additional support.
• Contribute to promotional activities and community outreach as needed.
• Report directly to the Medical Director and work closely with operational leadership to align efforts with organizational goals.

Patient Population / Client Focus

You will provide care primarily to older adults on Medicare, focusing on maintaining their health and enabling them to lead fulfilling lives. This patient population often has complex medical needs, and your role in creating personalized care plans and fostering trust is essential. Your work will contribute to the organization’s mission to enhance patient outcomes through meaningful engagement and proactive health management.

Qualifications

• Master of Science in Nursing, Doctor of Nursing Practice, or completion of an NP Certificate Program.
• National certification as a Family, Adult, or Adult-Gerontology Primary Care Nurse Practitioner from recognized bodies such as AANP or ANCC.
• Valid, non-probationary state Nurse Practitioner license.
• Active DEA license.
• Strong experience in a primary care setting, particularly in internal medicine, geriatrics, or family medicine.
• Familiarity with HCC documentation, ICD-10 coding, and health risk assessments.

Preferred Experience

Compensation and Benefits
