Location: Dublin / Radford, VA & Surrounding Areas (In-Home Patient Care)

Schedule: Full-Time / Part-Time / PRN (Flexible Scheduling Available)

Pay: Salary or Per-Visit Pay (based on experience)

Job Overview:
We are currently seeking a compassionate and skilled Home Health Physical Therapist – PT to provide personalized, one-on-one therapy in a personal setting throughout Dublin, Radford, and the surrounding coverage areas. This position is ideal for a Physical Therapist who enjoys working independently while helping patients regain strength, mobility, and functionality in their daily environment.

As a Home Health Physical Therapist – PT , you will work directly with patients recovering from illness, injury, or mobility challenges while developing individualized treatment plans focused on improving patient outcomes and quality of life.

Responsibilities of Home Health Physical Therapist – PT:

• Provide in house physical therapy to patients recovering from illness or injury
• Conduct patient assessments and develop individualized treatment plans
• Monitor patient progress and modify care plans as needed
• Assist patients with exercises and hands-on therapy to improve mobility and manage pain
• Educate patients on therapy goals, exercises, and self-care techniques
• Coach family members on safely assisting with exercises and patient care
• Collaborate with interdisciplinary team members including RN, LPN, CNA, HHA, OT, and SLP
• Maintain accurate and timely documentation of patient progress

Qualifications of Home Health Physical Therapist – PT:

• Graduate of an accredited Physical Therapist program
• Active PT license in the state of Virginia
• Minimum of 2 years of physical therapy experience (home health preferred)
• CPR certification required
• Knowledge of state and federal home health regulations
• Strong clinical judgment, interpersonal, and critical thinking skills
• Compassionate, dependable, and patient-focused
• Ability to work independently in a personal setting
• Willingness to travel throughout the service area
