Home Health Physical Therapist (PT) Per Diem | West Valley, Arizona Billet Health is looking for a compassionate and dedicated Physical Therapist (PT) to join our Home Health team in the West Valley. This per diem opportunity offers flexibility and autonomy while allowing you to provide one-on-one care that truly makes a difference. As a Home Health Physical Therapist, you will evaluate and treat patients in their homes, helping them regain mobility, improve function, and maintain independence. Responsibilities Perform comprehensive patient evaluations and assessments Develop individualized treatment plans based on physician orders Provide skilled physical therapy services in patients' homes Monitor patient progress and adjust treatment plans as needed Educate patients and caregivers on exercises, mobility, and safety Collaborate with physicians and interdisciplinary team members Complete accurate and timely documentation Qualifications Graduate of an accredited Physical Therapy program Current Arizona Physical Therapist license Valid driver's license and reliable transportation Current auto insurance CPR or BLS certification Current TB documentation (skin test within one year or chest X-ray within five years) Home Health experience preferred Compensation & Benefits Starting at $80 per visit Flexible scheduling Per diem position 401(k) Dental insurance Vision insurance Location: West Valley, Arizona Job Type: Per Diem If you're passionate about helping patients achieve greater mobility and independence, we'd love to hear from you. #J-18808-Ljbffr
