Key Responsibilities\n

• Conduct comprehensive and compassionate assessments of clients’ social, emotional, and environmental challenges to inform individualized service plans.

• Develop and implement service plans that target specific goals and interventions, regularly monitoring progress and adapting approaches as necessary.

• Provide individual and family counseling, utilizing evidence-based practices to address mental health concerns and improve overall well-being.
\n
• Engage in collaborative discussions with clients to identify their needs and connect them with relevant community resources, ensuring access to services.
\n
• Deliver crisis intervention when necessary, performing risk assessments and safety planning to protect clients’ well-being.
\n
• Work closely with community agencies and organizations to advocate for client needs and coordinate services to provide comprehensive support.
\n
• Participate actively in community outreach initiatives, raising awareness about services and addressing systemic issues affecting underserved populations.

• Contribute to case conferences and staff training, sharing insights and learning from peers to enhance effective practice.

Patient Population / Client Focus\n

Your work will primarily focus on serving vulnerable populations facing significant social challenges, such as those affected by poverty, homelessness, and mental health issues. You will empower clients from diverse backgrounds through individualized support, fostering resilience and encouraging positive life changes. By addressing both immediate crisis situations and long-term developmental goals, you will help clients to build healthier, more fulfilling lives.

Qualifications\n

• A Master's degree in Social Work from an accredited institution.

• Current active licensure as a Licensed Clinical Social Worker (LCSW) within New York State.

• Strong knowledge of social justice issues and familiarity with community resources available to clients.

• Exceptional interpersonal and communication skills, along with cultural competence in interacting with diverse populations.

• Proficiency in using case management software and other relevant tools to document and track client progress effectively.

Preferred Experience\n

• Prior experience working in a social service agency or mental health facility is preferred but not mandatory.

Compensation and Benefits\n
