Key Responsibilities\n

• Conduct comprehensive assessments to identify and understand the unique needs and challenges facing each patient.

• Engage in open communication with supervisors, colleagues, and family members to monitor and discuss patient progress effectively.

• Develop and share a weekly schedule with supervisors, detailing appointments, home visits, and other vital observations to track work contributions.

• Coordinate and execute in-home visits, facilitating direct engagement that aids in observations, chart reviews, and staff interviews to optimize patient care.

• Ensure adherence to treatment plans by regularly reviewing and refining approaches to maximize patient outcomes.

• Actively participate in interdisciplinary team meetings to exchange insights and refine collective strategies that enhance patient care.

• Document monthly and quarterly progress notes to provide a clear record of patient developments and care milestones.
\n
• Organize staff training sessions to ensure all team members are proficient in the program's implementation and documentation processes.

• Provide follow-up services through interviews and home visits, reinforcing support to meet ongoing patient needs.

• Maintain accurate records that facilitate easy access to information for authorized parties, ensuring compliance with regulations.

• Serve as a liaison between Rethink Mental Healthcare and the broader mental health community, fostering collaborative initiatives that enhance community wellness.

As an LPCC with Rethink Mental Healthcare, you will work with a diverse range of clients, including adults and youth from various backgrounds. Qualifications\n

• A Master's or Doctoral degree in Counseling, Psychology, or a related field.

• Current and unrestricted licensure as a Professional Clinical Counselor (LPCC) in California.
