Key Responsibilities\n

• Conduct comprehensive psychosocial assessments for patients and their families, identifying strengths and challenges related to their medical diagnoses, personal circumstances, and treatment processes.

• Develop and implement individualized treatment plans using appropriate social work interventions, ensuring a patient-centered approach in care delivery.

• Collaborate closely with healthcare professionals, integrating their insights into the treatment plans and maintaining open communication channels with patients and families to foster their involvement in care decisions.

• Lead discharge planning by working with various stakeholders, including physicians and insurance providers, to facilitate smooth transitions from the hospice setting to home or other post-acute care resources.

• Provide educational resources and counseling to help patients and families navigate emotional challenges associated with illness, adapting to new realities and enhancing coping mechanisms.
\n
• Engage in crisis intervention and implement brief or long-term therapy approaches, including individual, group, and family sessions as required by each unique case.
\n
• Evaluate oncology patients for psychological distress, such as anxiety and depression, and provide timely short-term counseling support.
\n
• Participate in an on-call rotation for after-hour support, ensuring continuous care availability.
\n
• Contribute to community outreach through educational presentations and supporting local cancer survivors' initiatives, enhancing community awareness and resource availability.

• Support the development of future professionals by mentoring interns and participating in training programs to advance the field of oncology social work.

Patient Population / Client Focus\n

The primary focus of this role centers on patients dealing with terminal illnesses, primarily within a hospice care framework. You will be instrumental in addressing the psychosocial needs of these individuals and their families, guiding them through their emotional and practical challenges during profoundly difficult times.

Qualifications\n

• A Master's degree in Social Work (MSW) from an accredited institution.

• Current and valid Licensed Clinical Social Worker (LCSW) registration in Colorado.
\n
• Minimum of two years of clinical social work experience, ideally in a healthcare or hospice setting.
\n
• Strong understanding of psychosocial theory, crisis intervention, and community resources pertaining to end-of-life care.

• Effective communication skills, both written and verbal, to liaise effectively with patients, families, and interdisciplinary teams.

Preferred Experience\n

• Experience in oncology social work or hospice care is highly desirable.

Compensation and Benefits\n
