Work Environment

This role is primarily onsite, meaning you will provide therapy services directly within patients' homes, which allows for a more personalized approach to care. Our organization values flexibility and work-life balance, offering you the ability to choose your own hours throughout the week (Monday to Friday) while focusing on patient-centered care.

Key Responsibilities

• Conduct thorough evaluations to assess patients' physical capabilities and limitations in their home environment.
• Establish comprehensive treatment plans that incorporate various therapeutic methods such as exercises, manual therapy, and assistive devices tailored to each patient's specific needs.
• Implement treatment strategies to alleviate pain, enhance strength and flexibility, and promote mobility.
• Document all patient interactions, treatment responses, and progress meticulously using our digital health record system.
• Regularly review and adjust treatment plans based on patient progress and evolving needs.
• Advise patients and families on the effective use of assistive devices, providing hands-on instruction and support.
• Engage actively in continuing education to keep abreast of the latest advancements in physical therapy practices.

Patient Population / Client Focus

Your clients will range from children to elderly patients dealing with a variety of conditions such as post-surgical recovery, chronic pain, and developmental challenges. By working in a home care setting, you will not only address the physical aspects of rehabilitation but also consider the emotional and environmental factors that can affect recovery.

Qualifications

• Bachelor's Degree in Physical Therapy from an accredited institution.
• Possession of a valid State License to practice as a Physical Therapist.
• Exceptional communication skills, ensuring clarity in patient interactions and documentation.
• Proficiency in technology, including electronic health records and patient management systems.
• Ability to be adaptable and demonstrate agility in various patient care scenarios.

Preferred Experience

• While we welcome new graduates, prior experience in home care or outpatient settings can be beneficial.
• Experience with specialized therapeutic techniques or patient populations is a plus.
