Key Responsibilities

• Conduct comprehensive assessments of clients' psychosocial needs, taking into account their emotional, mental, and social health.
• Develop and implement personalized care plans that reflect the clients' specific goals and address their unique challenges.
• Provide counseling and emotional support to seniors and their families, guiding them through significant life transitions, including health changes and loss of independence.
• Advocate for clients to secure access to necessary resources and services, ensuring they receive the full spectrum of support available.
• Collaboration is key; you will regularly engage with interdisciplinary teams, including nurses and healthcare professionals, to enhance and coordinate care effectively.
• Perform ongoing visits and check-ins through telehealth or in-person sessions to assess client well-being and address any safety or living condition concerns.
• Facilitate family communication sessions to resolve conflicts and promote understanding of care needs.
• Educate clients and their families about community resources and programs that can aid in their social engagement.
• Maintain meticulous records of client assessments, care plans, and any changes in client situations, using our electronic health record (EHR) system.
• Stay updated on best practices in social work, compliance standards, and ethical guidelines to ensure the highest level of care.

Qualifications

Compensation and Benefits

The salary range for this role is competitive, starting at $60,000 and reaching up to $100,000 per year, commensurate with experience and qualifications.
