Work Environment

This position is primarily remote, allowing for flexibility while you assist clients from the comfort of their own environments. However, you may occasionally need to meet face-to-face with clients or colleagues, especially for assessments, community outreach, and training. The hospice center setting may also require you to navigate sensitive areas of end-of-life care, whereby your role will be crucial in delivering compassionate support and bereavement counseling.

Key Responsibilities

• Conduct thorough assessments to evaluate clients' social, emotional, and environmental needs, identifying barriers to wellbeing.
• Design personalized service plans tailored to each client's goals, integrating a variety of interventions and strategies for progress monitoring.
• Provide individualized and family counseling that addresses diverse life challenges, ensuring a safe and supportive therapeutic environment.
• Engage clients actively in service planning, helping them connect with critical community resources that foster long-term stability.
• Utilize crisis intervention techniques, including risk assessments and safety planning, to support clients in high-stress situations.
• Collaborate with partner organizations and community agencies to coordinate comprehensive services and advocate effectively on behalf of clients.
• Participate in outreach initiatives to promote awareness of available agency services and support community involvement.
• Engage in regular case conferences and team meetings to discuss client progress, best practices, and ongoing professional development.

Qualifications

Compensation and Benefits
