Key Responsibilities\n

  Conduct comprehensive assessments to identify the specific needs and goals of patients during their ongoing treatment.

  Maintain clear and open communication between supervisors, direct care staff, family members, and other stakeholders to ensure patients receive cohesive support during their mental health journey.

  Develop and provide a detailed schedule for appointments, home visits, and ongoing observations to monitor patient progress.

  Coordinate home visits that include in-home evaluations, charting reviews, and data analysis, ensuring a holistic approach to patient care.

  Implement and evaluate treatment plans to ensure that interventions are effective and aligned with best practices in mental health treatment.

  Participate actively in Interdisciplinary Team Meetings, contributing insights and recommendations for improving patient care.

  Document and complete comprehensive monthly and quarterly progress notes, keeping an accurate account of patient development and outcomes.

  Facilitate training sessions to certify staff members on best practices related to program plans and documentation requirements.

• \n

  Provide follow-up interviews and observations to support the ongoing needs of clients post-treatment.

  Act as a liaison between the mental health community and the public, advancing community awareness and providing resources for mental health development goals.

  Contribute to the development of training materials and procedures aimed at enhancing program efficacy and staff competence.

Patient Population / Client Focus\n

In this role, you will work with a diverse patient population that includes both adults and youth experiencing a range of mental health challenges. Your work will encompass various therapeutic approaches, supporting individuals as they navigate through issues such as trauma, family dynamics, and substance misuse. We prioritize building trusting relationships with our clients, allowing for effective interventions tailored to their unique needs.

Qualifications\n

  A Master's or Doctoral degree in Counseling, Psychology, or a related field is required.

  An active and unrestricted licensure as a Professional Counselor (LPC) within Oregon.

  A minimum of two years of counseling experience in a clinical setting, providing direct patient care.

  Demonstrated proficiency in family therapy, couples therapy, trauma-informed care, or substance misuse treatment is essential.

  A solid understanding of legal and ethical standards within the realm of mental health practice.

  Exceptional communication and interpersonal skills that foster rapport and trust with clients.

  Proven ability to collaborate effectively within a multidisciplinary healthcare team.

  Familiarity with Electronic Medical Records (EMR) software and relevant technologies is preferable.

Compensation and Benefits\n

The salary range for this position is competitive, with an annual salary between USD 90,000 and 100,000, determined based on your experience and caseload capacity.
