Key Responsibilities\n

• Plan, coordinate, manage, and implement customized support packages tailored for clients and their families to help navigate the end-of-life process.

• Conduct thorough assessments of service users by interviewing them and evaluating their current mental and emotional health, needs, strengths, and vulnerabilities.
\n
• Develop individual treatment plans that set measurable goals specific to each client’s situation and needs.
\n
• Continuously monitor and evaluate client progress, making adjustments to treatment plans as necessary to ensure effective outcomes.

• Provide informed guidance and recommendation during counseling sessions, facilitating discussions around coping strategies, emotional support, and community resources.
\n
• Maintain comprehensive and accurate records of client interactions and progress, ensuring confidentiality and compliance with all regulations.
\n
• Engage in case conferencing and provide documentation and evidence in court when required, supporting clients’ needs throughout legal processes.

• Act as a key worker within a multidisciplinary team, collaborating closely with healthcare providers, nursing staff, and spiritual care teams to deliver integrated care.

• Refer clients and families to additional community services and support resources to assist in their recovery journey and enhance their support network.

Patient Population / Client Focus\n

Your primary focus will be on clients receiving hospice care, along with their families, who may be dealing with grief, anxiety, and complex emotional responses as they navigate the dying process. Your role will be to provide not only practical support but also emotional and psychological assistance to help clients find peace and dignity in their care, ensuring that their final days are as comfortable and meaningful as possible.

Qualifications\n

• A Bachelor’s degree in social work (BSW), psychology, sociology, or a related field is required; a Master's degree is preferred.

• A valid Licensed Clinical Social Worker (LCSW) credential.

Compensation and Benefits\n
