Full-Time Physical Therapist for Home Care Services

Work Environment

The successful candidate will work onsite, primarily in home care settings, engaging directly with patients in their own environments. This allows for more tailored treatment planning and effective interventions. Our team operates from Monday to Friday, from 8 AM to 3 PM, offering a stable schedule that promotes work-life balance. Key Responsibilities

• Conduct comprehensive evaluations of patients to assess physical function and develop individualized treatment plans based on their unique needs.
• Implement therapeutic interventions, utilizing techniques that may be adapted for delivery in-person or via telehealth when appropriate for the patient.
• Conduct ongoing assessments of patients' progress, analyzing clinical signs and adjusting treatment plans and goals as necessary to ensure optimal recovery.
• Document all patient interactions, including evaluations, treatment plans, and progress reports, using our electronic record system in compliance with established guidelines.
• Educate patients and their families about recovery strategies, rehabilitation techniques, and any lifestyle modifications required to support continued improvement.
• Collaborate with interdisciplinary colleagues to ensure holistic care for clients who may benefit from multiple therapy services.
• Proactively meet departmental performance goals and contribute to business objectives through proactive engagement and quality care delivery.

Patient Population / Client Focus

As a Physical Therapist at Pro Active Physio Clinic, you will work with a varied patient demographic that includes children and adults of diverse backgrounds, diagnoses, and age groups. Your focus will be on developing productive therapeutic relationships that empower patients to overcome physical challenges and enhance their daily functioning. This role particularly emphasizes responsiveness to the unique needs of each patient, adapting treatment modalities to meet individual goals.

Qualifications

• A Bachelor's degree in Physical Therapy is required; a Master's degree or Doctorate in Physical Therapy (DPT) from an accredited program is preferred.
• Current and unrestricted licensure as a Physical Therapist in the state of California.
• Willingness and capability to perform evaluations and therapies in both traditional settings and via telehealth when clinically appropriate.
• Strong commitment to serving a diverse population, demonstrating cultural competence and sensitivity.

Compensation and Benefits

We offer a competitive salary range of USD 100,000 – 120,000 annually, commensurate with experience and performance. In addition to a lucrative salary, our comprehensive benefits package includes:
