This position is fully remote, allowing you the flexibility to provide care from a comfortable setting while still being actively engaged with clients through virtual consultations. As a member of our hospice team, you will collaborate closely with medical staff, including physicians and nurses, to ensure that clients receive seamless, interdisciplinary support. Key Responsibilities\n

• Conduct comprehensive assessments and evaluations to identify clients' mental health needs, social determinants of health, and treatment goals. For instance, use standardized assessment tools to measure psychological distress and create a path forward.

• Provide individualized counseling sessions, addressing a broad spectrum of mental health and psychosocial issues, including grief, anxiety, and depression. Use evidence-based practices, such as Cognitive Behavioral Therapy (CBT), to effectively support clients.

• Facilitate therapeutic group sessions, offering a space for clients to share experiences, foster support networks, and engage in psychoeducation surrounding end-of-life issues.

• Develop and implement client-centered treatment plans, setting measurable objectives and choosing interventions based on best practices and client needs.

• Deliver crisis intervention services, including performing risk assessments, creating safety plans, and coordinating with emergency services when necessary.

• Advocate for clients' rights and facilitate access to healthcare and community resources, ensuring they receive the comprehensive support they need.

• Maintain detailed and confidential client records that comply with legal and ethical standards, including treatment plans, progress notes, and outcome assessments.

Your primary focus will be on clients receiving hospice care, many of whom face terminal illnesses and end-of-life transitions. Qualifications\n

• Master's degree in Social Work (MSW) from an accredited institution.

• Current licensure as a Licensed Clinical Social Worker (LCSW) in the state of Tennessee.

Compensation and Benefits\n

We offer a competitive salary ranging from USD 80,000 to USD 95,000 annually, commensurate with experience and qualifications.
