Key Responsibilities\n

As a Mental Health Therapist at Elevance Counseling LLC, your role will encompass a wide range of responsibilities, including but not limited to:

• Conducting psychosocial assessments to accurately diagnose and treat various mental, emotional, and behavioral disorders, and modifying care plans in response to each patient's evolving needs.

• Developing comprehensive treatment plans in collaboration with the multidisciplinary team to ensure continuity of care, and effectively coordinating discharge plans for optimal recovery.

• Providing family and individual therapy sessions that are tailored to the unique circumstances and backgrounds of each patient to enhance their coping strategies and emotional resilience.

• Facilitating referrals to community services, support groups, or additional specialists (e.g., psychiatrists) as part of a holistic approach to wellbeing.
\n
• Acting as an advocate for patients and their families, ensuring clear, effective communication among all parties involved in the patient’s care.

• Documenting progress in a timely manner using our streamlined digital platform, which supports efficient note-taking and case management.

• Participating in team meetings and case consultations to share insights and improve the overall standard of care across the organization.

• Identifying clients at risk for avoidable delays or complications and working proactively to mitigate those risks in collaboration with care coordinators and other professionals.

Qualifications\n

Compensation and Benefits\n
