Work Environment\n

This role is primarily remote, allowing for flexibility while still being an integral part of a community health clinic team. You will operate within an innovative digital framework, utilizing telehealth systems to engage with patients. While the role is predominantly remote, occasional in-person meetings may be required for team collaboration, training, or community outreach events.

Key Responsibilities\n

• Conduct initial assessments and comprehensive screenings to determine the mental health needs of patients, utilizing standardized tools and clinical judgment to develop individualized treatment plans.

• Provide evidence-based individual counseling services, focusing on short-term strategies that empower patients to address immediate challenges.

• Facilitate therapeutic groups and workshops on topics such as anxiety, depression management, and coping strategies, fostering a sense of community among participants.

• Collaborate closely with our medical providers, social workers, and support staff to create an integrated approach to patient care, ensuring holistic treatment pathways.

• Participate in community outreach initiatives, including mental health awareness campaigns, to educate the public and reduce stigma associated with mental health issues.

• Maintain accurate, confidential records and documentation, adhering to all ethical and legal guidelines while using our case management software.

Patient Population / Client Focus\n

Our patient population is diverse, consisting of individuals and families from various backgrounds who may experience a range of mental health concerns, from mild anxiety to more severe emotional disturbances. We emphasize cultural competency and are committed to understanding and addressing the unique socio-economic and demographic factors affecting the mental health of our clients.

Qualifications\n

• Possession of an active state license as a Licensed Marriage and Family Therapist (LMFT), Licensed Mental Health Counselor (LMHC), Licensed Independent Clinical Social Worker (LICSW), or Psychologist.

• A minimum of a Bachelor’s Degree in Counseling, Psychology, Social Work, or a related field; a Master’s or Doctoral degree is preferred.

• In-depth understanding of mental health challenges specific to community health settings, including access barriers and social determinants of health.

• Strong interpersonal and communication skills to effectively connect with a diverse patient population and engage in multidisciplinary teamwork.

• Proficiency in evidence-based practices and familiarity with telehealth platforms for service delivery.
