Work Environment\n

This position will be primarily remote, allowing for flexibility in managing work-life balance while maintaining a strong connection with patients through virtual consultations and support. The successful candidate will be expected to operate within set work hours, specifically from Monday to Friday, 9:00 AM to 5:00 PM. While predominantly remote, occasional in-person meetings and team collaborations may take place at our San Jose Hospice Center.

Key Responsibilities\n

As the Licensed Clinical Social Worker at Riverplace Health System, you will be tasked with:

• Providing individualized psychotherapy to patients experiencing mental health conditions, such as anxiety, depression, and post-traumatic stress disorder (PTSD).

• Conducting comprehensive assessments of each patient's mental health status, covering emotional, behavioral, and social functioning.

• Crafting and implementing evidence-based treatment plans tailored to the unique needs and cultural backgrounds of each patient, ensuring their treatment is both relevant and effective.

• Collaborating closely with interdisciplinary healthcare teams, including psychiatrists and primary care physicians, to promote a cohesive treatment strategy and optimal patient outcomes.

• Monitoring patient progress, adjusting treatment plans as required in response to patient feedback and clinical assessments.

• Educating patients and their families about mental health conditions, available treatment options, and community resources to enhance overall understanding and well-being.

Qualifications\n

The ideal candidate will possess:

Compensation and Benefits\n
