Work Environment

This role is primarily onsite at local schools, where collaboration with a multidisciplinary team is key. As a Nurse Practitioner, you will work closely with physicians, physician assistants, and medical staff including medical assistants, a dedicated medical scribe, registered nurses, and case managers. This collaborative care environment fosters a team-oriented approach to patient care, enhancing the quality of service delivered.

Key Responsibilities

• Provide outstanding primary care tailored to individual patient needs, adapting the approach based on holistic assessments.
• Conduct routine and acute office visits, effectively diagnosing and managing a variety of health concerns.
• Perform annual wellness visits and comprehensive health risk assessments to ensure proactive care management.
• Coordinate care with other providers, facilitating referrals to specialists and arranging necessary diagnostic tests.
• Lead population health initiatives within the care team, ensuring routine screenings and preventive services, such as biannual breast cancer screenings for eligible patients.
• Assist the care team with phone triage and patient outreach to ensure continued engagement and follow-up.
• Educate patients about their health conditions, treatment plans, and self-management strategies to empower them in their care journey.
• Engage in promotional activities to advocate for community health initiatives and organizational goals.
• Conduct home visits when necessary, providing care and support directly in the community.
• Report directly to the Medical Director and collaborate closely with operational leadership for continuous improvement.

Patient Population / Client Focus

Your primary focus will be on older adults within the Medicare population, a demographic that often requires specialized care management. The goal is to improve patient quality of life through tailored, accessible health services that nurture long-term relationships and trust. By emphasizing continuity of care, you will help ensure that patients not only understand their care plans fully but also feel actively involved in their healthcare decisions.

Qualifications

• A Master of Science in Nursing, Doctor of Nursing Practice, or completion of a Nurse Practitioner Certificate Program.
• National certification as a Family, Adult, or Adult-Gerontology Primary Care Nurse Practitioner through the American Association of Nurse Practitioners (AANP) or American Nurses Credentialing Center (ANCC).
• An active, non-probationary Nurse Practitioner license in the state of Nevada.
• A current DEA license for prescribing controlled substances.
• Substantial experience in primary care—particularly in internal medicine, geriatrics, or family medicine—and familiarity with population health management.
• Proficient understanding of HCC (Hierarchical Condition Categories), ICD-10 coding, and Health Risk Assessment guidelines.
• A strong team player with a desire to engage in collaborative care approaches and innovative models of practice.
• Bilingual candidates are preferred to assist our diverse patient population.

Preferred Experience

Experience in outpatient settings and familiarity with electronic health records (EHR) systems will be a significant advantage in delivering effective patient care. Applicants with leadership experience in quality improvement initiatives or population health management will also be viewed favorably.

Compensation and Benefits

• Competitive salary ranging from $120,000 to $160,000 annually, with opportunities for adjustment based on experience and specialized skills.
