Key Responsibilities\n

• Conduct thorough evaluations and assessments to understand the needs of each child and their family.

• Participate actively in the development and implementation of Individualized Family Service Plans (IFSP), ensuring family involvement and understanding of strategies.

• Collaborate with a multidisciplinary team to monitor and report on outcomes related to child development and family support.

• Provide educational and practical support to families, empowering them with skills to enhance their child’s development.

• Demonstrate compliance with all relevant federal and state regulations concerning social work practice.

• Build strong rapport and trust with children and families, fostering a safe and encouraging learning atmosphere.

• Use evidence-based practices and Natural Learning Environment Practices when working with families.
\n
• Implement IFSPs effectively, tailoring the frequency and type of services based on developmental assessments and family-centered priorities.

• Utilize coaching techniques in natural settings to promote developmental growth in children.

• Adhere to safety and ethical standards to protect the well-being of staff and the individuals we serve.

Patient Population / Client Focus\n

Your primary focus will be on young children who are currently enrolled in hospice care, along with their families. This population often experiences complex emotional and developmental challenges, and you will play a critical role in advocating for and supporting their unique needs. By leveraging your skills and experience, you will help families navigate difficult circumstances, promoting a supportive and nurturing environment for their child’s overall well-being.

Qualifications\n

• Master's Degree or Doctorate in Social Work from an accredited institution.

• Current licensure as a Licensed Clinical Social Worker (LCSW) recognized by the Texas Department of Public Health.

• Proven experience of at least one year working with children in early intervention or related settings.
