Key Responsibilities\n

• Conduct thorough psychosocial assessments using interviews, observations, and medical record reviews to identify emotional, social, and environmental factors affecting patient recovery.

• Provide individual and family counseling sessions that tackle adjustment disorders, emotional distress, and coping mechanisms associated with terminal illness.

• Collaborate with an interdisciplinary rehabilitation team to create individualized treatment plans that integrate psychosocial insights into patient care.

• Assist patients and their families in navigating community resources, including but not limited to support groups, vocational rehabilitation services, and housing assistance programs.

• Educate patients and their families about available post-rehabilitation services and how to access necessary support post-discharge.

• Contribute to discharge planning processes, ensuring a seamless transition to home or alternative care settings, including addressing any psychological readiness for discharge.

Compensation and Benefits\n
