Key Responsibilities\n

As a Licensed Clinical Social Worker within our hospice setting, you will assume a variety of critical responsibilities:

• Conduct comprehensive clinical assessments to evaluate patients' psychological and social needs, using a variety of evidence-based tools and methodologies.

• Develop tailored treatment plans, incorporating solution-based interventions and individualized therapeutic approaches that are respectful of each patient’s unique circumstances and values.

• Implement and facilitate therapeutic interventions, including cognitive-behavioral therapy (CBT) and other modalities suited for individuals coping with serious illness, loss, and grief.

• Maintain accurate, timely, and confidential clinical records in strict adherence to HIPAA regulations, ensuring that patient privacy is always protected.

• Engage in regular interdisciplinary team meetings to discuss patient cases, share insights, and coordinate care effectively among team members.

• Educate and empower patients and their families about coping strategies, available resources, and community support systems, enhancing their journey through hospice care.

This role specifically focuses on the needs of patients residing in hospice care, many of whom are facing terminal illnesses. Qualifications\n

To be considered for this position, you must meet the following qualifications:

Compensation and Benefits\n

This full-time position offers a competitive salary range of USD 80,000 to 100,000 annually, commensurate with experience.
