Legacy Healthcare Services is searching for an experienced and motivated Physical Therapist (PT) to join our newly created Better at Home team. Pay Rate: Based on a Per Visit Rate

Coverage Location: Wake County, NC

Responsibilities:

• Provide physical therapy services to patients according to physician's plan of care and as defined in the state Physical Therapy Practice Act.
• Assess and evaluate therapeutic, rehabilitative/functional status to develop, implement and direct physical therapy programs that focus on rehabilitation.
• Evaluate the patient's home environment, assess patients for muscle strength, mobility, gait, ROM potential for rehab.
• Direct patients and families/caregivers in the use and care of therapeutic appliances.
• Initiate physical therapy program and instruct other personnel and/or family/caregiver members in certain phases of physical therapy with which they may work with a patient.
• Identify patient and family/caregiver needs for other home health (PCA) services and refer as necessary.
• Prepare and submit progress summaries based on the attainment of goals.
• Supervises PTAs according to organization policy and state regulations.

Position Requirements:

• Degree in area of discipline
• Current Physical Therapy license in state of practice, or ability to obtain prior to beginning employment
• Excellent interpersonal, fiscal, and clinical skills
• Computer proficiency
