Nurse Care Coordinator – Full Time – Onsite – Atlanta, GA Location: Atlanta, Tucker, GA, US. Position Type: Full Time, Weekdays, Onsite. Reports to: Practice Manager. Summary The Nurse Care Coordinator provides both in‑clinic and tele‑care services for clients facing an unplanned pregnancy. The role includes non‑diagnostic telephone and in‑person assessment, education, and support, and collaborates with Human Coalition to facilitate rescue of children and support families. Responsibilities Evaluate client situation by phone, identify concerns, and provide education on abortion, parenting, and adoption services. Connect women with necessary resources to alleviate pressure from circumstances. Provide limited medical services to women facing unplanned pregnancies. Act as a key member of the onsite medical team offering confidential, compassionate care for pregnant mothers. Offer counseling, education, and emotional support to clients and their families. Document all client interactions and execute care plans accurately and timely in the electronic Client Management System (CMS). Communicate pertinent information to the Medical Director following organizational policies. Intervene to promote client safety and security in compliance with professional nurse practice standards. Demonstrate patient‑centered behaviors and handle both medical and non‑medical questions with independent judgment. Perform additional duties as assigned. Education, Certifications and Experience Current LVN or RN nursing license in good standing, or equivalent compact license. 2–4 years of prior clinical experience in any specialty (preferred). Emphasis on obstetrical and gynecological care preferred. Current CPR certification for healthcare professionals. Supervisory Experience None. This role is an individual contributor.
