Key Responsibilities\n

• Conduct thorough clinical evaluations and assessments to develop individualized treatment plans tailored to each patient's unique needs.

• Manage case coordination activities, including communication with the Utilization Review and Business Departments for necessary authorizations and payment processing.

• Initiate discharge planning on the day of admission, ensuring that all patients have a clear understanding of their treatment goals and exit strategies.

• Hold discharge sessions with patients to solidify their coping strategies, transition plans, and resources that will support their ongoing recovery after leaving the program.

• Confirm discharge appointments with patients and their families, ensuring that they are equipped for a successful transition out of care.

Patient Population / Client Focus\n

In this role, you will work primarily with adults experiencing substance use disorders and co-occurring mental health issues. Qualifications\n

Compensation and Benefits\n

We offer a competitive annual salary ranging from USD 120,000 to 130,000, aligned with your experience and caseload.
