Key Responsibilities

• Conduct comprehensive evaluations by reviewing patient history, performing physical assessments, and determining necessary interventions while documenting findings in a clear and detailed manner.
• Effectively communicate evaluation results and treatment plans to patients and their families, involving them in setting realistic, attainable goals.
• Implement treatment protocols tailored to individual patient needs, ensuring compliance with best practices and adapting as necessary based on patient progress and evolving healthcare standards.
• Collaborate with a multidisciplinary team of healthcare professionals, recognizing when a patient's conditions require alterations to their care plan or referrals for additional medical interventions.
• Coordinate discharge planning and aftercare, ensuring a seamless transition from rehabilitation back to daily activities, and maintain accurate, meticulous records of patient encounters in accordance with regulatory requirements.
• Manage your schedule with effective time management practices, meeting productivity expectations while also participating in community outreach and marketing initiatives to promote our services.
• Contribute to departmental goals through participation in strategy meetings and compliance with federal and state regulations governing rehabilitation services.
• Engage in personal and professional development by pursuing continuing education opportunities, as well as mentoring and instructing students in clinical placements.
• Foster a patient-centered approach by promoting accessibility and adaptability in care delivery and actively working to overcome barriers that affect patient access to services.

Patient Population / Client Focus

In this role, you will work with a diverse group of patients ranging from post-operative rehabilitation cases to individuals managing chronic conditions. Our facility caters to patients of various ages, each with unique rehabilitation needs. By employing individualized treatment plans that merge clinical expertise with compassionate care, you'll help enhance patients' mobility, reduce pain, and improve their overall quality of life.

Qualifications

• A Bachelor's Degree in Physical Therapy from an accredited institution.
• Must possess a valid Physical Therapist license in the State of New Jersey.
• Current certification in Basic Life Support (BLS) for Healthcare Providers by the American Heart Association.
• Excellent communication and interpersonal skills, essential for working closely with patients and the healthcare team.

Preferred Experience

• A minimum of two years of clinical experience as a licensed physical therapist, preferably in a rehabilitation setting.

Compensation and Benefits

This position offers a competitive salary ranging from $100,000 to $120,000 annually, commensurate with experience.
