Director of Social Services – Make a Difference Every Day

McKnight Place Extended Care, an award-winning senior living community in St. Louis, is looking for a Director of Social Services to support residents and families while leading a comprehensive psychosocial care program. This role is key in advocating for resident needs, guiding families, and fostering a positive, supportive environment.

What You'll Do:

• Lead resident service plan meetings and maintain psychosocial care documentation
• Support residents and families with resources and resident rights
• Educate staff and families on mental health, dementia, and related topics
• Complete assessments (MMSE, GDS, FAST) for new residents
• Refer residents to community agencies when needed
• Serve as weekend Manager on Duty on rotation

Qualifications:

• Bachelor's degree in Social Work Required (Master's preferred).
• 1+ year in senior living, skilled nursing, or hospital setting required.
• Strong communication, organization, and teamwork skills.
• Experience with Medicare Part A processes, including skilled nursing documentation, care coordination, discharge planning, and regulatory compliance.
