Key Responsibilities\n

• Conduct individualized and group counseling sessions, assessing patient needs and tailoring therapy accordingly, using modalities such as cognitive behavioral therapy and motivational interviewing.

• Complete thorough assessments and provide necessary case management services; this includes triage, crisis intervention, and coordinating care for referral clients.

• Identify and implement suitable outreach activities aimed at enhancing community wellness initiatives and promoting mental health awareness.

• Participate actively in multidisciplinary team meetings, contributing to case consultations and case coordination alongside clinical and medical staff to ensure a holistic approach to patient care.

• Document patient interactions accurately in our electronic health records system, ensuring compliance with all regulatory standards and quality metrics.

In this role, you will support a diverse range of clients primarily within the hospice setting, including patients dealing with terminal illnesses and their families. Qualifications\n
