Key Responsibilities:

• Prepare and dispense medications for home infusion therapy accurately and safely
• Conduct thorough reviews of medication orders and patient histories
• Monitor patient responses to therapy and provide clinical support as needed
• Collaborate with healthcare providers and patients to optimize treatment outcomes
• Maintain compliance with pharmacy laws, regulations, and standards
• Document all pharmacy-related activities in patient records

Qualifications & Desired Experience:

• Licensure as a Pharmacist with current authorization to practice
• IV Certification and demonstrated proficiency in home infusion pharmacy operations
• Previous experience in home infusion or related pharmacy practice
• Strong knowledge of medication management and patient counseling
• Excellent communication and organizational skills
• Ability to work independently and handle per diem assignments with flexibility
