Key Responsibilities\n

As a Licensed Clinical Social Worker at CHDC, your primary responsibilities will include:

• Delivering clinical behavioral health services that align with CHDC’s mission and quality standards, ultimately promoting improved health outcomes and increased patient satisfaction.

• Navigating mental health consumers through the complexities of physical and behavioral healthcare systems to enable them access to integrated healthcare services.

• Educating patients on available resources and services, which encourages proactive participation in their healthcare journey, disease prevention, and lifestyle wellness.
\n
• Engaging in regular multi-disciplinary treatment team meetings to discuss patient care strategies and contribute clinical insights.
\n
• Ensuring the timely documentation and reporting of crisis incidents to the Medical Director and Chief Medical Officer, while adhering to organizational protocols.
\n
• Maintaining detailed clinical records encompassing patient histories, treatment plans, and progress notes to ensure continuity of care.
\n
• Participating in ongoing Quality Improvement initiatives by attending meetings, contributing feedback, and partaking in training sessions.
\n
• Applying evidence-based screening tools to evaluate the healthcare needs of patients and communicate findings efficiently.
\n
• Collaborating with the Center of Excellence Team to assist patients with Opioid Use Disorder, providing specialized support and resources tailored to their needs.

Patient Population / Client Focus\n

The LCSW will work primarily with a diverse patient base that includes individuals struggling with mental health issues, substance use disorders, and socio-economic challenges. Our focus is on creating an inclusive environment where all patients feel heard, respected, and supported in their recovery and wellness journeys. Qualifications\n

• A master’s degree in social work from an accredited institution with an active Licensed Clinical Social Worker (LCSW) credential.

• At least two years of social work experience, with a focus on clinical settings.

• Willingness to travel between CHDC sites for meetings, training, and outreach functions as required.

Preferred Experience\n

Experience working in integrated behavioral health settings, with proficiency in evidenced-based practices and familiarity with the latest therapeutic modalities is preferred. An understanding of the healthcare systems and community resources in the Pittsburgh area will enhance the effectiveness of your role.

Compensation and Benefits\n

This full-time position offers a competitive salary ranging from USD 90,000 to 100,000 annually, commensurate with experience and qualifications.
