Key Responsibilities\n

• Conduct comprehensive assessments and evaluations to identify patient needs and tailor appropriate treatment plans.

• Maintain clear communication with supervisors, the care team, families, and other stakeholders regarding clients' progress and treatment developments.

• Develop a detailed schedule for appointments, home visits, and observations to effectively monitor each client's journey.
\n
• Facilitate in-home visits, conducting observations, chart reviews, and interviews to ensure the accurate information is gathered for treatment plans.

• Ensure that all therapeutic interventions are executed according to the documented program plans, actively participating in ongoing evaluations.
\n
• Engage in Interdisciplinary Team Meetings alongside clinical peers to align treatment goals and strategies across disciplines.
\n
• Document patient progress through detailed monthly and quarterly notes, ensuring compliance with legal and ethical standards.
\n
• Lead training sessions for staff on the implementation of program plans and necessary documentation procedures to enhance team performance.
\n
• Conduct follow-up interviews and maintain ongoing support for patients to ensure their needs are met beyond initial assessments.

• Serve as a point of contact for the mental health community, representing the organization at public events and fostering partnerships to enhance resource availability.

Patient Population / Client Focus\n

In your role, you will work with a diverse clientele including adults and youth, addressing a wide range of mental health concerns. You will provide culturally competent care to individuals facing challenges such as trauma, substance misuse, and relationship issues, all while focusing on empowering them to achieve their personal health goals.

Qualifications\n

• A Master's or Doctoral degree in Counseling, Psychology, or a related discipline is required.

• An active and unrestricted license as a Professional Counselor (LPC) in the state of Arizona.
\n
• A minimum of two years of experience in a counseling role, specifically within a clinical environment.

• Familiarity with different therapy modalities, including family therapy, couples therapy, and trauma-informed care is a plus.

• Deep understanding of legal and ethical standards in counseling practices, ensuring the protection and advocacy of client rights.

• Exceptional interpersonal skills with the ability to build rapport with clients from various backgrounds.

• Proficiency with Electronic Medical Records (EMR) software and related technologies.
