Key Responsibilities\n

• Conduct comprehensive assessments for new and ongoing clients, tailoring treatment plans to address individual needs effectively.

• Maintain continuous communication with guardians, family members, relevant stakeholders, and interdisciplinary team members regarding clients’ progress and treatment strategies.

• Provide supervisors with a structured schedule of appointments, home visits, and sessions to monitor therapeutic outcomes and accountability.

• Create and coordinate home visits, utilizing strategies for in-home observations, chart reviews, and data analysis that informs treatment adjustments.

• Implement and ensure the effective execution of program plans while conducting regular reviews and updates based on client feedback and outcomes.

• Engage in Interdisciplinary Team Meetings to discuss client cases, share best practices, and identify areas for program improvements.

• Document monthly and quarterly progress notes to track advancements and inform treatment adjustments.

• Organize and facilitate training for team members and staff on implementing treatment plans and maintaining documentation standards.

• Provide ongoing follow-up interviews and observations, ensuring long-term support for clients’ evolving needs.

• Act as a liaison to the public and mental health community to drive initiatives that align with the organization’s development goals.

Qualifications\n
