Key Responsibilities\n

Reporting directly to the Social Work Manager, your responsibilities will include:

• Conducting thorough bio-psychosocial evaluations to assess and identify barriers to treatment, ultimately enhancing patient health outcomes.

• Developing personalized treatment plans that include both individual and group therapy modalities, while regularly evaluating and adjusting approaches based on patient progress.

• Providing consultation to a multidisciplinary team regarding patients' psychosocial needs to ensure comprehensive care that promotes better treatment adherence.

• Documenting all clinical activities in an efficient and timely manner, maintaining adherence to departmental standards and all relevant laws and regulations.

• Educating patients and their families on therapeutic processes and community resources, fostering understanding and engagement in their own care.

• Advocating for patient needs and coordinating with various community resources to enhance social support and overall wellness.

• Completing case-related statistics and reporting to inform ongoing quality assurance and organizational improvements.

Patient Population / Client Focus\n

In this role, you will primarily serve a diverse patient population seeking mental health services within the community health clinic framework. You will address a variety of psychosocial problems that may arise from mental illness, helping clients navigate personal and environmental challenges that impact their overall functioning. Qualifications\n

The ideal candidate for this position will possess:

• A Master’s Degree in Social Work from an accredited institution.

• A current LCSW license from the State of Illinois.
