This position is primarily onsite, focusing on home care settings across Boca Grande, Florida. Key Responsibilities

• Conduct thorough evaluations and assessments of patients referred by physicians to establish a clear treatment plan.
• Implement customized treatment programs that encompass a range of therapy techniques and modalities including therapeutic exercise, manual therapy, and functional training.
• Document progress and adjust treatment plans based on patient performance, ensuring alignment with the physician's prescribed plan of care.
• Demonstrate exceptional customer service skills, reinforcing our commitment to patient-centered care.
• Collaborate with an interdisciplinary team, facilitating communication to maximize patient outcomes through joint efforts and shared assessments.
• Consult and provide education to patients and their caregivers, empowering them in their recovery journey and promoting independence in daily activities.
• Achieve set utilization goals while supporting the overall operational objectives of the organization.

Patient Population / Client Focus

This role serves a diverse patient population, including orthopedic, pediatric, neurologic, and sports medicine clients. Our therapists are equipped to address a wide variety of conditions, from post-surgical rehabilitation to chronic pain management, thus requiring adaptability and a deep understanding of individualized treatment approaches.

Qualifications

• A Doctorate degree in Physical Therapy (DPT) from an accredited institution is required.
• Must possess a valid State License as a Doctor of Physical Therapy (DPT) issued by the Physical Therapy Examining Committee.
• At least two years of relevant clinical experience demonstrating the ability to effectively treat a variety of patient conditions.
• Current CPR certification and familiarity with safety protocols in home care settings.
• Strong written and verbal communication skills for optimal patient interaction and documentation efforts.
• Proficient in word processing and electronic health record systems to ensure seamless administrative processes.
• Commitment to building strong therapeutic relationships that empower patients and foster positive outcomes.

Preferred Experience

Experience working within home care environments and familiarity with telehealth practices to enhance patient engagement and access to care.

Compensation and Benefits

This position offers a competitive salary ranging from USD 100,000 to USD 120,000 annually, complemented by an attractive benefits package. Our benefits include:
