Work Environment

You will be working onsite, primarily in the comforting setting of patients' homes, where you'll have the opportunity to make a substantial impact on their rehabilitation journey. Our team collaborates closely, ensuring continuity of care and effective communication among all healthcare professionals involved in a patient's treatment. The typical work schedule for this position is Monday through Friday, from 8:00 am to 5:00 pm. This structured schedule allows you to engage in comprehensive patient care without weekend commitments.

Key Responsibilities

• Conduct thorough assessments of patients with functional limitations, gait dysfunction, and musculoskeletal pain, establishing individualized treatment plans accordingly.
• Employ evidence-based treatment protocols tailored to the unique needs of each patient, ensuring optimal recovery and rehabilitation outcomes.
• Document patient histories, evaluations, and treatment progress meticulously in compliance with healthcare documentation standards.
• Engage in collaborative discussions with other healthcare professionals to adjust patient treatment plans as necessary, responding to changes in patients' conditions.
• Implement discharge planning and provide detailed follow-up instructions to ensure continuity of care post-therapy.
• Maintain compliance with all relevant regulations and organizational policies while upholding best practices within home care settings.
• Contribute to team meetings by sharing insights on patient care and participating in initiatives aimed at improving access to therapy services.

Patient Population / Client Focus

Your primary focus will be on patients recovering from orthopedic surgeries, sports injuries, and chronic pain conditions. You will address a variety of issues that impact mobility and function. By utilizing an empathetic, personalized approach, you will build essential relationships that foster trust and cooperation throughout the rehabilitation process.

Qualifications

• Doctorate of Physical Therapy (DPT) from an accredited institution.
• Current state licensure to practice as a Physical Therapist in Florida.
• A robust understanding of state practice acts and compliance regulations affecting physical therapy practice.

Preferred Experience

• A minimum of two years of practical experience in outpatient orthopedic physical therapy.
• Current certification in CPR and BLS.
• Ability to demonstrate proficiency in the latest rehabilitative technologies and treatment approaches.

Compensation and Benefits
